The shape of our nose can change with increasing age. The reason for this lies in two different, age-related mechanisms:
1) As we age, the structural stability within our nose can decrease. The loss of support leads to a long, and thin nose with a droopy nasal tip, where the tip of the nose is pointing downwards. Frequently patients complain about an increasingly longer nose. A droopy tip and an elongated nose can be accompanied with an impairment of nasal breathing. Quite often, patients describe an improvement in airway passage, when they lift up the tip of their nose.
2) A typical sign of aging is the continuous loss of volume, especially under our eyes and our cheeks. Based on its anatomy, the nose can maintain its height, whereas the surrounding areas of the face continuously descent. As the soft tissue of the cheeks flatten, the nose begins to appear larger, and a mild dorsal hump can suddenly be more prominent, a normal sized nose can suddenly begin to look disproportionally big. The loss of facial volume can not only exaggerate the appearance of the nose, but equally aggravate the appearance of an aging face.
Both mechanisms, the elongation of the nose and the loss of facial volume disrupt the balance and harmony of the face. During consultation, it is therefore imperative to perform a meticulous physical exam of the nose and an equally detailed evaluation of the proportions of the patient’s face. The right approach, might involve a rhinoplasty, or a treatment of the midface or both.
