The process of aging is a multidimensional process. Traditionally plastic surgery was only tackling the sagginess by pulling the tissues upwards thus combating only the effect that gravity had to the face. However, by further studying the complicated process of aging we now understand that structural changes also happen, including the fat on our faces.

Facial liposculpture and fat grafting rejuvenates the face by giving back the lost volume and removing excess fat from unwanted areas by a very gentle liposuction. Fat grafting can be done alone or combined with other facial procedures, such as eyelid surgery, facial liposuction or face and neck lifts. Fat grafting also called fat transfer or lipofilling is used in Micro-fat to sculpt the face and return the lost volume and as nano-fat to improve the quality of the skin.

Procedure

The fat is harvested from another area such as the lower inside part of the thighs or occasionally abdomen in a very gentle way and under very low suction to preserve the integrity of the fat cells, then it is very gently centrifuged to separate the liquid and oil, after its emulsified and filtered to insure a homogenous quality and then injected with special cannulas in specific marked areas of the face, from forehead, eyebrows, temples, upper eyelid, lower eyelid, nasolabial folds, corners of the mouth and cheek, hands or wherever needed. From the same harvested fat, some fat is designated to be broken down with specific instruments, and all the vital contents of the fat cells are freed, it is then filtered and the whole skin of the face, neck, décolletage and even hands can be treated to improve the skin quality.

Length

One to two and a half hours.

Anaesthesia

Depending on the patient and the treatment either local anaesthesia and sedation or general anaesthesia.

INPATIENT/OUTPATIENT

Outpatient procedure or day case.

POSSIBLE SIDE EFFECTS

Some swelling is common which subsides gradually within the first two weeks, some bruising can also happen especially if nano-fat treatment is also performed.

RECOVERY

After a week only mild swelling and bruising may be present which will subside in the following week or two. In case its combined with liposuction of the lower face, wearing a compression garment will be mandatory for the first couple of weeks.

RESULTS

The results are long lasting. Some fat which is used for volume replacement will inevitably get absorbed during the first 4-6 weeks. The rate of absorption depends on many factors especially the areas treated. For example, the areas around the eyes and chin have the lowest rate of absorption and about 80% of the grafted fat will survive and the forehead and cheek areas about 50% of the fat will survive. The fat that has survived will stay long-term, but the natural changes of aging will continue.
